Use of an Individual Number Card as a Health Insurance Card

With the integration of the Individual Number Card and the Health Insurance Card, the Health Insurance Card will be abolished, and you will be able to use your Individual Number Card (Individual Insurance Card) that you have registered as your health insurance card to receive medical care.

Transitional Measures for the Abolition of the Health Insurance Card

Although new Health Insurance Cards will no longer be issued from December 2, 2024, Health Insurance Cards already issued can be used until December 1, 2025, as a transitional measure.
Note that if you lose your health insurance eligibility due to reasons such as retirement, the Health Insurance Card will no longer be usable after the date of loss of eligibility.

How to Apply for an Individual Number Card

You must apply to receive your Individual Number Card.

How to Register your Individual Number Card as a Health Insurance Card

In order to use your Individual Number Card as your health insurance card, you must complete the necessary procedures in advance using the Individual Number Portal, etc.

*Please submit a cancellation application form to the health insurance society if you wish to cancel your registration for using the Individual Number Card as a health insurance card.
The cancellation application form is published on the "Application Forms" page.
Due to the procedures involved, it will take one to two months from the time the cancellation application is received until the registration is canceled. You can check the completion status of the cancellation from "Status of Application for Use of Individual Number Card as health insurance card” in the Individual Number Portal (Myna Portal).

How to Check Health Insurance Eligibility Information

Issuance of "Eligibility Notification（資格情報のお知らせ）"

The health insurance society will issue an Eligibility Notification so that you can easily check your own health insurance enrollment status, etc.
Please keep this Eligibility Notification in a safe place, as you will need to include your health insurance code and number on any application forms you submit to the health insurance society.

Checking Information from the Individual Number Portal eligibility information screen

If you have a smartphone, you can check your health insurance eligibility information (health insurance code and number, etc.) from the Individual Number Portal eligibility information screen.

*If you have not submitted your Individual Number to the Health Insurance Society, or if the submitted Individual Number is being verified by the Health Insurance Society, you will not be able to check your eligibility information from the Individual Number Portal.

When the card reader at a medical institution or pharmacy is not functioning or when you are receiving medical care at a medical institution or pharmacy that does not support some Individual Number Insurance Cards, you can present your Individual Number Insurance Card along with the Eligibility Notification or the Individual Number Portal eligibility information screen to receive medical care.

*Only presenting the Eligibility Notification or the Individual Number Portal eligibility information screen alone is insufficient for receiving medical care at a medical institution.

How to Use Your Individual Number Insurance Card (Procedure at Medical Institutions)

When you Cannot use Your Individual Number Card

When the card reader at a medical institution or pharmacy is not functioning or when you are receiving medical care at a medical institution or pharmacy that does not support Individual Number Insurance Cards, you can present your Individual Number Insurance Card along with the Eligibility Notification or the Individual Number Portal eligibility information screen to receive medical care.

Eligibility Confirmation Document （資格確認書）

If you do not have an Individual Number Card or have not registered your Individual Number Card for use as a health insurance card, you can receive medical care at a medical institution or pharmacy by presenting the "Eligibility Confirmation Document （資格確認書）" issued by the health insurance society.
Although an "Eligibility Confirmation Document" will be issued without having to apply if you do not have an Individual Number Insurance Card, because it takes a considerable amount of time to confirm the usage status of the Individual Number Insurance Card, please apply for one when you acquire eligibility, etc. if you do not have an Individual Number Insurance Card.

Eligibility Confirmation Document Has an Expiration Date

Please return the "Eligibility Confirmation Document" if you lose eligibility before the expiration date (returning is unnecessary if you lose eligibility after the expiration date).

Handling of Individual Numbers at the Works Applications Group Health Insurance Society

Personal information which includes individual numbers is personally identifiable information and requires even more strict handling than personal information. The law restricts personally identifiable information from being used or disclosed even with the consent of an individual.
Health insurance societies are designated as persons in charge of processes using individual numbers and may only use individual numbers within the administrative scope stipulated by the Act on the Use of Numbers to Identify a Specific Individual in the Administrative Procedure.

The Social Security and Tax Number System

The Social Security and Tax Number System assigns an individual number to every person with a certificate of residence and uses personal information that exists across multiple government agencies to confirm data about an individual in order to efficiently manage data in social security, tax, and disaster recovery matters.

The Japanese government adopted this social security and tax number system with the aim of achieving a fair and just society, enhancing public convenience, and developing administrative efficiency.
